Dangerous Goods Incident/Accident Report form (F2914) Purpose of the form
In the event of a dangerous goods incident that results in a dangerous situation, it is a requirement of section 150(3) of the Transport Operations (Road Use Management – Dangerous Goods) Regulation 2008 that the prime contractor advise the Department of Transport and Main Roads of the situation followed by a detailed written report.
The incident may be during transport or during loading or unloading operations.
The Dangerous Goods Incident/Accident Report form (F2914) has been approved for the reporting of any such incident. However, the reporting person(s) may attach additional information to the form if necessary.
Definition of a dangerous situation
A 'dangerous situation' is a situation involving the transportation of dangerous goods that is causing or is likely to cause imminent risk of:
- death of, or significant injury to, a person
- significant harm to the environment
- significant damage to property.
Reporting requirements
As soon as possible after the incident, the prime contractor must tell the Department of Transport and Main Roads (either verbally or in writing) within 24 hours after becoming aware of the incident:
- where the incident happened
- the time and date of the incident
- the nature of the incident
- the dangerous goods being transported when the incident happened
- any other details the chief executive may reasonably require.
As well, within 21 days after the day the incident happens, the prime contractor must give the Department of Transport and Main Roads a written report about the incident stating the following:
- where the incident happened
- the time and date of the incident
- the nature of the incident
- what the driver of the vehicle believes to be the likely cause of the incident
- what the prime contractor believes to be the likely cause of the incident
- the dangerous goods being transported when the incident happened
- the measures taken to control any leak, spill or escape of dangerous goods, and any fire or explosion, arising out of the incident
- the measures taken after the incident in relation to the dangerous goods involved in the incident.

Dangerous goods drivers must contact us within 24 hours of an incident to provide details of the incident.
