Labels, registration and intercept books
Follow these steps to request replacement labels and intercept books or to change vehicle registration numbers:
- Complete and sign the National Heavy Vehicle Accreditation Scheme Request for Replacement Labels/Intercept books (Mass and Maintenance)/Advice of Change of Registration form (F4529).
- Post to
NHVRAS Administrator
Department of Transport and Main Roads
PO Box 673, Fortitude Valley 4006
or scan the signed form and email it to NHVAS@tmr.qld.gov.au. No payment is required.
Note:
- Replacement labels:
The above form can be used to order replacement large (prime mover) or small (trailer) labels or the mass or maintenance strips for the large labels.
- Intercept books:
Please ensure when ordering these books that all company details, vehicle registration numbers and the number of intercept books required are specified. Only the intercept books that are required should be ordered (no spare books will be sent out).
