Changing your name or address

If you hold a Queensland driver licence, you must notify us within 14 days if you change your name or address.

How do I notify a change of name?

If you have changed your name, you need to:
• visit a customer service centre
• complete an application form and provide evidence of identity

For more information about acceptable evidence of identity documents please refer to the Evidence of Identity Information Sheet (F4362).

Note: If you have changed your name, you will be issued a new licence card which will display your new name. You will need to hand in your current licence when you apply. If you are unable to hand in your current licence, a replacement fee will apply.
