Cairns tilt train derailment

Shortly after departing Berajondo, at about 2351 when passing through the first of a series of speed curve restrictions, the co-driver left his seat and entered the adjacent vestibule area to tidy up and make a 'brew' for the driver.

After departing Berajondo, at about 2354 having passed through a series of eight curve restrictions, the co-driver left his seat and entered the adjacent vestibule area to tidy up and make a 'brew' for the driver.
Page 22, 2.8 Train Management, third paragraph, last three sentences to be changed from:
On clearing Berajondo, the co-driver left his seat and entered the adjacent vestibule area to make a 'brew' for the driver, this was three or four minutes before the derailment. He was in the adjacent vestibule when the train derailed. He was therefore not in a position to observe the speed boards at 415.663 km, 416.480 km, 417.783 km and the critical speed for 419.410 km and intervene as may have become necessary.
