Surrender a driver licence

You may voluntarily surrender your drive licence at any time.

If you have a medical condition that prevents you from driving and you wish to voluntarily surrender your driver licence, you may complete either a Medical Condition Notification form F4355, or a Return/Surrender Declaration form F4778 and return it along with your driver licence to your nearest transport and motoring customer service centre, or mail it to:

Note: If you voluntarily surrender your driver licence, or have it cancelled because of a medical condition then you may be eligible to receive an Adult Proof of Age card free of charge as an alternative form of photo identity. Enquire at a transport and motoring customer service centre, or call 13 23 80 for more information.

If you wish to voluntarily surrender your driver licence for any other reason, you may complete a Return/Surrender Declaration form F4778 and return it along with your driver licence to your nearest transport and motoring customer service centre, or mail it to:

Note: If your driver licence is still current, or has expired within the last 2 years, you  may use it as evidence of identity to apply for an Adult Proof of Age card before you surrender it. An Adult Proof of Age card is an ideal form of photo identification for people who don’t hold a driver licence. 

Refund of licence fee

If your driver licence is current, you may be eligible for a refund of the unexpired portion. A refund only applies if you are surrendering a licence:

  • voluntarily due to a medical condition
  • because your licence has been cancelled due to a medical condition
  • on behalf of a deceased person.

You are not eligible for a refund if you voluntarily surrender your licence for any other reason, or if you have been disqualified from holding a driver licence.

Medical condition

If you are eligible for a refund you may apply either in writing or on the Medical Condition Notification form F4355. In some instances, you may be required to provide:

  • a copy of your Licence Surrender Receipt
  • a copy of your medical certificate.

On behalf of a deceased person

To obtain a refund you must apply in writing and provide:

  • your name and address (refunds will only be issued by cheque in the name of the licensee or the licensee's estate)
  • a copy of Licence Surrender Receipt 
  • a copy of 1of the following:
    • Death Certificate, Funeral Notice or Coroner's Report 
    • Certificate of Authority issued by Public Trustee under the Public Trustee Act 1978.
