Purchasing modification certificate books and modification plates

To purchase modification certificate books and/or modification plates, we require the following procedures to be followed. These procedures are in place to ensure the security of the sale and distribution of modification certificate books and modification plates.

Approved Persons must only use modification certificates and plates issued to them. You must not allow anyone else to use modification plates or certificates that have been issued to you, even if you work for the same business or employer. Doing so may put you at risk of being held responsible for modifications certified by another person.

Procedure for approved persons residing in Queensland

Note: Modification certificate books and plates cannot be purchased from Transport and Main Roads’ Redbank Customer Service Centre.

When purchasing modification certificates and/or modification plates, you are required to provide the following to a departmental Customer Service Centre or Queensland Government Agency Program (QGAP) office:

• completed Application to Purchase Books of Certificates of Modification and/or Modification Plates
• the required fee (refer to the approved person fee schedule)
• your current approved person accreditation document
• your evidence of identity for individuals (form S5385).

You may nominate another person to purchase the modification certificates and/or modification plates on your behalf. This can be done by completing the authorisation to act on my behalf section of the Application to Purchase Books of Certificates of Modification and/or Modification plates. The nominated person must also produce the evidence of identity of the nominated person (form S5385) to the nearest departmental Customer Service Centre or Queensland Government Agency Program (QGAP) office.

From 1 November 2019, a new maroon modification plate must be issued to certify S10 -concessional livestock loading – vehicle rating modifications. To purchase maroon modification plates you must lodge a completed Certificate of Modification and/or Modification Plates application form (F1856) at your nearest customer service centre or Queensland Government Agency Program (QGAP) office. The plates will be mailed to you by registered post to the postal address nominated on the application form.

Procedure for approved persons residing interstate (outside of Queensland)

If you reside interstate, you can purchase books of certificates of modification and/or modification plates by mail. To purchase books of certificates of modification and/or modification plates, you must:

• complete Application to Purchase Books of Certificates of Modification and/or Modification Plates
• provide a cheque or money order for the required fee (refer to the approved person fee schedule)
• provide a copy of your approved person accreditation document
• your evidence of identity for individuals (form S5385)
• post the above documents to:
