Supporting documents for an approved person application
In addition to providing copies of your industry experience and qualifications, you may be required to provide additional documents to support your application (supporting documents) when making an approved person application, making an application to add additional modification codes or renewing your approved person accreditation.
Professional membership
If you are making an application for a modification code that requires qualification number 1 (refer to the approved person qualification table) and requires membership with one of the listed professional engineering bodies, you must provide a copy of your current membership with your application. You must also provide evidence of your current professional membership when renewing your approved person accreditation or when making an application for additional modification codes.
Statement of Attainment
If you are making an approved person application for a modification code and hold a trade certificate issued since the introduction of national policy for regulated qualifications (implemented from 2000), you must also provide a Statement of Attainment provided by the registered training organisation (RTO). For more information about trade certificates and qualifications, refer to the approved person qualifications.
Street Rod certification (modification codes LH9 and LH10)
In addition to providing copies of your industry experience and qualifications, if you are applying for street rod modification codes LH9 and/or LH10 you must also provide copies of:
- a letter of confirmation of your training and eligibility for street rod modification codes LH9 and/or LH10 from the Queensland Street Rod Technical Advisory Committee Association Inc.
- your current Australian Street Rod Federation membership.
You must also provide a copy of your Australian Street Rod Federation membership when renewing your approved person accreditation.
